Person Centred Team Based Care

Leading the establishment of team-based care, where a spectrum of team members work collaboratively with patients,  families,  communities, and each other to achieve shared goals, where people are empowered to drive their care to the extent they wish, and where all health care team members work to the full extent of their education, certification, and experience.​

Team-based care is the provision of health services to individuals, families, and/or their communities by at least two health providers who work collaboratively with patients and their caregivers—to the extent preferred by each patient— to accomplish shared goals within and across settings to achieve coordinated, high-quality care.

Team-based care improves patient outcomes, patient satisfaction, staff satisfaction and overall engagement of staff.

The Three Factor Framework is used to describe the circumstances in relation to the patient population, the health care professional group, and the environment.  Each circumstance is of equal importance and must be consider when discerning the most appropriate model of care.
